Nice summary. According to the Chaos Report about 70% of all projects fail so BPM projects are following the pattern of other projects.

In my experience one of the main reasons for BPM project failure has been the desire for over complication of the solution. Business and / or IT does not set up for multiple iterations in what I call a "crawl, walk, run" experience. Someone always wants to start running before they learn to crawl.
